are now coordinated with the electronic records system that is completed at orientation and/or before your clinical/practicums and you will always have access to them in your account of the electronic medical records if you should need them in the future after your first year of completing them.

However, during the transition anyone who does not have an electronic health records started because they are a current student following the old process can find the forms on the Background Check & Health Records section located on the left sidebar of the Current Student page (this page).
